It is the policy of Highcliffe Residential Home that a Care Plan is drawn up on each resident's admission. This is known as a Residents Care Plan.

The Care Plan is devised by a member of the management team and in consultation with you and/or your family. This plan is tailored to meet your requirements and will be re-assessed constantly to make sure that any changes in circumstances are noted and changed if necessary.

Each resident is assigned a key worker. The key worker will "befriend" you and undertake to review all your needs monthly. This will be done with you and/or your family. The key worker may undertake to review more frequently should your circumstances merit this.

Any information that is held at Highcliffe Residential Home regarding yourself is confidential and can be seen at your request by yourself or designated family member.
